Jeff had been skinned alive.

The doctors and nurses were all in shock when they saw Jeff's face.

One of the doctors working at the mental hospital Jeff was sent to was fed up with his repeated misbehavior and uncooperativeness. He was also fed up with the way the other doctors failed to get Jeff's mental state back to normal. And so, the only solution that he could think of was to take his scalpel and slice off all of his bleached-burned facial skin and leave him to bleed to death.

But that got him into serious hot water.

The doctor that had ripped off Jeff's face was scorned for his actions and fired from his job. The doctors and nurses that remained tried as best as possible to fix up Jeff's scalped and scarred face and stop the bleeding.

One doctor, a skin culturist wanted to make a new face for Jeff. However, he did not have enough replacement skin to make it. So instead, he took all of the scalped remains of Jeff's old face ( most of it was all still in one piece ) and made some modifications to it so that he could wear his old face like a mask.

The mask also had some special clips that helped keep his face in place.

Once the skin culturist had the mask completed and the doctors and nurses stopped Jeff's face from bleeding, the mask was then put on and clipped into place. And although some of his bare muscle tissue was still not covered up and his bare teeth still exposed, Jeff liked his " new look ".

The doctors and nurses were still in shock when they saw Jeff put on the mask. Since it used the scalped remains of Jeff's old face, it still had that bleached white color. Because the mask had no lips, Jeff's bare teeth still showed. And since it had no eyelids either, Jeff still had that always-open-eyed stare with the bloodshot eyes. The exposed muscle tissue in some parts ( like the neck ), the metal fastener clips and the leather strap that held his face in place only added to Jeff's inhumaness.

But most importantly, it still had that infamous, dreadful smile that they had come to know and dread. It was bigger now and more revealing.

The skin culturist would later go on to make a brand-new face for Jeff using brand-new replacement skin after he helped him escape from the asylum.
